A lockshield valve never reopened

The valve at the far end of a radiator is usually closed to take it off and it is easy to forget to open it again, or to open it only part way. The radiator then gets little or no flow and stays cold while everything else works.

Air left in the circuit after refilling

Draining part of a system to remove a radiator lets air in, and refilling without venting properly leaves it there. In a flat with high ceilings the air often ends up somewhere higher than the radiator it is affecting, which is why bleeding that radiator achieves nothing.

A thermostatic head refitted backwards

These have an orientation and fitting one the wrong way round means it senses the pipe rather than the room, so it closes early and that room never warms up. It looks correct from across the room and it is a two minute fix once spotted.

Pressure dropped and never topped back up

Removing a radiator takes water out of a sealed system, and if it was not brought back to pressure afterwards the boiler may be running low or cutting out entirely. That is the one on this list most likely to be mistaken for an appliance fault.

Almost certainly something to do with the radiators coming off rather than the boiler. The usual suspects are a lockshield valve closed to lift the radiator and never reopened, air left in the circuit after refilling, or a thermostatic head put back the wrong way round.
